Why napkins?

If we're trying to find problems with the costs, and napkins are not mention in the case, how are we suppose to ask about napkins in specific?

Short answer: You're not necessarily.

Brainstorming questions aren't always about getting the exact right answer. Rather you need to show structured thinking (i.e. think about a few key themes) and show an ability to have good ideas.

In this particular case, the interviewer is supposed to tell you about this new opportunity - there's no way you could possibly guess about it!
