What are typical operations cases (aka process improvement case) ?

Root cause analysis / driver trees

Does anyone have experience with ops cases that focus on root cause analysis & driver trees. While these concepts are important in MBB cases, they rarely get fully tested, but ops consulting really tests these concepts. I'm looking for some material on these. if someone has experience with this please get back to me.
