In terms of the others, if we keep the companies that just do strategy and are not related to Big 4 with their brand name (ie, we don’t consider Accenture, Deloitte, EYP), then you get Strategy&, Kearney, Roland Berger, Oliver Wyman in the first positions. For companies that also do non-strategy projects, such as Deloitte, identifying a proper ranking is more tricky, as candidates might join for different reasons/positions.
This is still an imprecise way to measure prestige as some companies might simply hire fewer MBA candidates than others due to preference or size, but could provide a general orientation of what top MBA candidates consider better for their career at that stage.
Keep in mind that geography and industry matter a lot in the definition of a top company. Examples:
- Strategy& can be considered a top player in the Middle East thanks to the Booz legacy, less so in other geographies
- Oliver Wyman can be considered a top player in financial service and has a very good reputation in terms of quality of work in that industry, less so in other sectors

I would not place Margin Improvement as a subset of Strategy, but as a subset of operations consulting.
For Strategy, the typical MBB > Tier 2 > Big4 applies. As such, Deloitte would probably be in the Top10, but not in the Top 5
For Operations Consulting, probably Deloitte would be a bit higher. This won't be different between Germany and the US.
Hmmm, pretty narrow topic! Deloitte is a Big4 / Tier 2. In most categories it will rank in this range.
Monitor is a top ten firm.
There are regional differences, but not major ones in terms of how it ranks.
If you're interested specifically in margin improvement, I'd recommend you reach out specifically to consultants who work in that space to get their perspective on it.
